Requirements: Microsoft Word 97 or better
Instructions:
1. Please download the file from the following link: layspeaker.zip
2. Change the filename from layspeaker.zip to layspeaker.doc
3. You should be able to open the document with Microsoft Word on you home PC. It can be printed and filled out by hand or can be filled out as a form in Microsoft Word and then printed.
4. If you don't have Microsoft Word, then click this link: layspeakertext.zip. Once again, you will need to change the name from layspeakertext.zip to layspeakertext.txt. Can then be printed and filled out.
(Note for advanced users: These aren't zip files. Extensions changed to force IE or Netscape to download as a file)
